Discharge from a hospital or skilled nursing facility often comes with little warning. The care team decides a patient no longer needs to be there, and the family has days to find somewhere safe. It is one of the most stressful moments a family can face.
We work alongside the discharge planner or case manager. We find communities and care homes with real openings that can meet the person’s needs, arrange quick assessments, and help you compare options while the clock is running. Clinical decisions stay with the treating team.

Paying for it
Medicare can cover a short stay in a skilled nursing facility after a qualifying hospital stay, but it generally doesn’t pay for assisted living. If longer-term care is needed, we help you understand private pay, ALTCS and VA options so a fast decision is still an informed one.
Common questions
Can you help if we live out of state?
Yes. We can tour on your behalf, send photos and notes, and keep you updated by phone so you can decide from wherever you are.
Do you work for the hospital?
No. We work for your family. We are glad to coordinate with the hospital team, but our job is to find the right place for your loved one.
