These terms apply to your use of this website and of the senior living placement service provided by Prime Care Placement ("we", "us"). By using the website or the service, you agree to these terms.
Service disclosure
Our service is free to families. When someone moves into a community we introduced, that community pays us a referral fee. It never changes the price your family pays.
Our placement service is provided at no cost to families and to the individuals seeking care. We are compensated by senior living communities and care homes with which we have referral agreements. A referral fee is typically payable to us only after a resident referred by us moves in. The fee is paid by the community and does not change the rate charged to the resident.
We may also share information about communities with which we do not have a referral agreement. Upon request, we will tell you whether we have an agreement with any community we discuss with you.
Nature of the service
We provide information and assistance to help families identify and evaluate senior living options. We do not provide medical care, personal care or nursing services, and we do not own, operate or manage any community or care home.
We are not a medical, legal, financial or insurance adviser. Information we provide about programs such as the Arizona Long Term Care System (ALTCS), Veterans Affairs benefits, Medicare or insurance is general in nature. Eligibility for any program is determined solely by the agency or insurer that administers it.
Your decisions
The choice of a community or care home is made by you and the person receiving care. Each community makes its own admission decisions, typically following its own assessment. You are under no obligation to choose any community we suggest.
We make reasonable efforts to provide accurate, current information about pricing, availability, services and licensing. However, this information is provided by third parties and changes frequently. We encourage you to confirm all information directly with the community and to review state licensing and inspection records before making a decision.
Use of this website
The content of this website is provided for general information only and does not constitute professional advice. You agree not to misuse the website, interfere with its operation, or submit information that is false or that you are not authorized to share.
Limitation of liability
To the fullest extent permitted by law, Prime Care Placement is not liable for the acts or omissions of any community, care home or care provider, or for any loss arising from your reliance on information provided by third parties.
Governing law
These terms are governed by the laws of the State of Arizona.

We may update these terms from time to time. The date at the top of this page shows when they were last revised.
