    Step 1 of 5

    Start with a conversation

    We begin with a call, or a visit if that’s easier. Tell us how the day goes now: bathing, dressing, medications, meals, getting around, and what happens at night. Then tell us what they enjoy and what they’d hate to give up.

    We’ll also talk about budget, the part of town that makes sense for family, and how soon a move needs to happen. If you don’t know some of it yet, that’s fine. We’ll fill in the gaps together.

    We make the calls

    Next we contact the communities and care homes that could be a fit, and we ask what their websites leave out: whether there is an opening, what the base rate and care fees really come to, what level of care their license allows, and who is on shift overnight.

    We also look up each one’s state inspection record. Anywhere that can’t meet the person’s needs is off the list before you hear about it.

    Review a short list

    You get a handful of real options, not a directory to wade through. For each one we explain what it does well, what it will cost each month, and anything that gives us pause.

    We’ll tell you which we’d look at first and why. You decide which ones to see, and nothing is booked until you say so.

    Visit in person

    We schedule tours around your calendar, grouped so you aren’t driving across the Valley twice in one day. We can come along, and we’ll give you a checklist so each visit is judged the same way.

    If the person moving can join, even for one visit, it helps. Afterwards we talk it through while the details are fresh.

    Settle in

    Once you choose, we stay involved. We help coordinate the community’s assessment, the paperwork and the move-in date, and we keep track of what each side still needs.

    After the move we check in with you. If something isn’t right in the first weeks, tell us. We’ll raise it with the community, or help you look again.

A brochure and a tour show you the best of a place. These are the things that decide whether it will actually work.

  • License and inspections

    The license type, the care it allows, and the state inspection record.

  • Care they can actually give

    Medications, transfers, diabetes, oxygen, memory support and nighttime help.

  • The real monthly cost

    Base rate, care fees, and the extras that show up on the first bill.

  • Staffing, day and night

    Who is on shift, how many, and what happens after a fall at 2 am.

  • Openings and timing

    Whether there is a room now, and how soon a move could happen.

  • Room to change

    What happens if needs increase, and whether they would need to move again.

What we are, and what we aren’t.

We are a placement and advocacy service. We don’t provide care, and we don’t own or run any of the communities we suggest. Each community makes its own admission decisions.

We aren’t medical, legal or financial advisers. When a question needs one, we’ll say so and help you find the right person.

Common questions

How long does this take?

It depends on how soon a move is needed. When a hospital or rehab discharge is close, we work to have options for you within a day or two. When you are planning ahead, we go at your pace.

Who makes the final decision?

You and your loved one do. We give you information and our honest opinion. Each community also decides whether it can meet the person’s needs, usually after its own assessment.

Do we have to choose one of the places you suggest?

No. There is no obligation at any point. If you find somewhere on your own, we’re still glad to share what we know about it.

What does it cost?

Our service is free to families. When someone moves into a community we introduced, that community pays us a referral fee. It never changes the price your family pays.
